Acknowledgement of Responsibilty *- Form used when a parent or guardian is unable to accompany their child to a team or association event. This form allows the parent to assign a Guardian who will being responsible for the custody of the Player and authorize the Guardian to approve any necessary emergency medical treatment
Advertising Form *- 2016 - Sponsor form for Program Book, Web Site, and Dasher Board advertising.
Bylaws *- The Bylaws of Junior Everblades Hockey Association, Inc. are hereby restated and adopted in their entirety, effective as of July 11, 2011.
Incident Report *- The JEB Incident Report has been instituted to allow members the ability to submit concerns or comments about incidents or issues they may have concerns about. This form will enable a member (Player, Parent/Guarding or Coach) to submit their concerns in writing without issuing a
formal Grievance Complaint. You may identify yourself or submit the form anonymously.
Member at Large Application *- Duties & Responsibilities of Member at Large: Individual will serve as a Member at Large for a period of one year before being eligible for consideration as a Board of Director of the JEB organization. Members are expected to attend all meetings unless prior arrangements are made with either the President or Vice President to be excused. Member's input will be considered on all matters relating to the organization; however, only BOD members are allowed to vote on issues.

Parent & Player Handbook Agreement *- Parent and Player agree to read the policies and procedures set forth in the Junior Everblades Parent & Player Handbook and agree to abide by them.
Transfer Form (International except Canadian) *- 2015 - Per International Ice Hockey Federation (IIHF) transfer regulations non-citizen players, men or women, under 18 years of age may be transferred by means of a letter of approval. If the player is 18 or over, contact USA Hockey for the appropriate transfer forms prior to regular season games. This form is to be used for non-US Citizens playing hockey on Youth & Girls classification teams. There is no fee to process a written transfer.
Ice Request Form *- 2016 - 2017 Form used when requesting unscheduled additional ice time, swapping scheduled ice time with another team, or canceling scheduled ice time. (requires 30 days advance notice)
